Terms and Conditions of Participation

New Frontiers in Interventional Cardiology (NFIC) Conference

I. General Provisions

  1. These Terms and Conditions define the rules of participation in the cardiology conference “NFIC – New Frontiers in Interventional Cardiology” (hereinafter referred to as the “Conference”).
  2. The event is organized by the Fundacja Ośrodek Badań Medycznych, based in Książ Wielki, Poland (address: ul. Dr. J. Danysz 3, 32-210 Książ Wielki, NIP: 6751307268, tel. +48 536 200 917, e-mail: biuro@fobm.pl), hereinafter referred to as the “Organizer.”
  3. A “Participant” of the Conference may be any natural or legal person who has completed registration and paid the registration fee in accordance with these Terms and Conditions.

II. Registration

4. Registration is carried out by completing the registration form available on the website www.nfic.pl.

5. Successful registration requires:

a. correctly completing the registration form,

b. acceptance of these Terms and Conditions, and Privacy and Cookie Policy,

c. payment of the registration fee as determined by the Organizer.

6. Upon successful registration, a confirmation message will be displayed on the website, and the Participant will receive an email confirmation.

7. Submitting the registration form via the Conference website constitutes acceptance of these Terms and Conditions, Privacy and Cookie Policy, as well as agreement to comply with all organizational rules and any additional arrangements between the Participant and the Organizer.

III. Registration Fee

  1. Prices and payment terms for participation are published on the Conference website.
  2. The registration fee includes:
    a. access to conference sessions and workshops,
    b. coffee breaks and lunch boxes,
    c. an electronic participation certificate, provided that the Participant attends the Conference in person.
  3. The registration fee must be paid within the period indicated on the proforma invoice sent to the email address provided during registration. Failure to make payment by the due date may result in cancellation of the registration.
  4. Once the payment has been received, the Organizer will issue a VAT invoice based on the billing details provided in the registration form.
  5. The VAT invoice will be issued in accordance with applicable tax regulations and will apply a 0% VAT rate.

IV. Cancellation Policy

  1. The Participant has the right to cancel their participation in the Conference.
  2. Cancellations must be submitted in writing to the email address provided on the Conference website.

Refund conditions are as follows:

a. cancellations received up to 30 days before the event – full refund of the registration fee,

b. cancellations received up to 15 days before the event – 50% refund of the registration fee,

c. cancellations received less than 15 days before the event – no refund.

15. Failure to attend the Conference does not entitle the Participant to a refund.

16. The Organizer reserves the right to make changes to speakers or the program due to circumstances beyond its control. Such changes do not entitle the Participant to a refund.

V. Complaints

  1. Any complaints regarding the Organizer’s performance should be submitted by email to biuro@fobm.pl.
  2. Complaints must be submitted no later than 14 days after the end of the Conference.
  3. Complaints submitted after this period will not be considered.

VI. Final Provisions

  1. The Organizer reserves the right to make changes to the Conference program for valid organizational or unforeseen reasons. Such changes do not constitute grounds for a refund of the registration fee.
  2. In the event that the Conference is cancelled for reasons attributable to the Organizer, the Participant is entitled to a full refund of the registration fee.
  3. The Organizer shall not be liable for any damages resulting from cancellation or rescheduling of the Conference due to circumstances beyond its control (force majeure).
  4. Communication with the Organizer shall take place via the contact details provided on the Conference website.
  5. By participating in the Conference, the Participant grants the Organizer the free, perpetual, and unlimited right to record, use, and distribute their image and voice captured during the event for documentation and promotional purposes.
  6. Participants are required to comply with the internal regulations of the venue where the Conference is held.
  7. The Organizer shall not be responsible for any loss, damage, or theft of Participants’ personal belongings during the event.
  8. Participants are financially liable for any damage they cause.
  9. In matters not regulated herein, the provisions of the Polish Civil Code shall apply.
  10. Personal data provided in the registration form shall be processed in accordance with the Polish Personal Data Protection Act (Dz.U. 2018, item 1000) and Regulation (EU) 2016/679 (GDPR). The information obligation in accordance with the GDPR constitutes Annex No. 1 to these Regulations.
  11. These Terms and Conditions come into effect upon publication on the Organizer’s website.

Annex No. 1 – Information on Personal Data Processing

  1. The controller of your personal data is Fundacja Ośrodek Badań Medycznych, with its registered office in Książ Wielki, ul. Dr Jadwigi Danysz 3, 32-210 Książ Wielki, Poland, entered in the Register of Associations, Other Social and Professional Organizations, Foundations, and Independent Public Health Care Institutions of the National Court Register maintained by the District Court for Kraków–Śródmieście in Kraków, 11th Commercial Division, under KRS No. 0000201727, REGON: 356818070, NIP: 6751307268; email: biuro@fobm.pl (hereinafter referred to as the “Controller”).
  2. The Controller has not appointed a Data Protection Officer. However, any questions regarding the processing of your personal data may be addressed directly to the Controller via e-mail: biuro@fobm.pl
  3. Your personal data are processed by the Controller based on your consent to the processing of your image and voice recorded during the Conference sessions in materials documenting the course of the event. The data collected in this way may be used for informational, archival, and promotional purposes related to the activities of the Organizer. The legal basis for processing is Article 6(1)(a) in conjunction with Article 9(2)(a) of the GDPR.
  4. Your personal data will be stored until you withdraw your consent to the processing of your image and voice. After this period, your data will be permanently deleted.
  5. You have the right to access your personal data and the right to request their rectification, deletion, restriction of processing, data portability, and to object to processing — to the extent permitted by applicable law. Requests in this regard should be addressed to the Controller.
  6. If you have given consent for the processing of your image, you have the right to withdraw this consent at any time. Withdrawal of consent does not affect the lawfulness of processing carried out prior to its withdrawal.
  7. You have the right to lodge a complaint with a supervisory authority if you believe that the processing of your personal data violates the provisions of the General Data Protection Regulation (GDPR).
  8. To ensure proper organizational operations (including accounting, IT, and administrative services), your personal data may be disclosed to the following categories of recipients:
  9. service providers supplying the Controller with technical or organizational solutions (in particular IT service providers);
  10. legal, accounting, auditing, and consulting service providers supporting the Controller in pursuing lawful claims (including law firms and financial institutions);
  11. other public authorities, institutions, or entities authorized under applicable law.
  12. The Controller will not make any automated decisions, including those based on profiling, in relation to your personal data.
  13. Personal data may be transferred to countries outside the European Economic Area (EEA), in particular to the United States, where Google and Vimeo servers are located. In such cases, the Controller applies appropriate safeguards, such as standard contractual clauses approved by the European Commission.

Legal basis: Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation).
